Creation of Ecosystem
The community will be divided in to clusters, and each cluster will be allocated with a specific number of villages.
All the referred cases will be monitored on their health conditions regularly and their progress of wellbeing is captured in EMR
Each household in the village will be mapped through door-to-door visit. The details of the family members of each household will be entered with a specific questionnaire and will be digitalized. Each household will be given HHMR (House Hold Medical Record) which will provide the entire medical history of each member of the family
Reports of the screened members in the household will be further investigated and potential ailments will be identified. Members requiring immediate attention will be referred to nearest & appropriate government health facility.
Every Member in the household will be screened on Targetted health conditions & EMR of every member will be created.
